Not everyone. It stand true most of the time, but some have not returned after death.

I don't have much. Some people who die stay that way. There does not seem to be a rhyme or reason between resurrection or final death that I've seen, but I also have not researched the subject extensively.

There were grave markers for those who had come and gone, but whether it is currently intact or not given our current climate is up for debate.
